How do I know if my doctor is a covered provider with my carrier?
You can check your provider directory supplied by your carrier. You can also use the carrier’s on-line provider directory by clicking the carrier’s logo on our Benefit Forms section. We will be happy to search for you as well.

I have not received my ID card yet. Am I covered?
In most cases you are. ID cards are usually generated within 5-10 business days after you have been entered into their system. ID cards are mailed directly to your home address. If you do not receive your card within this timeframe, please notify your employer’s administrator.

How can I receive a new medical ID card if mine is lost?
Most carrier websites provide a place to print a temporary card and also request a new card. Click on our Benefit Forms section and click on your carrier’s logo to log on.

Are there forms on-line that I can use to add an employee on to our medical benefits?
An Enrollment Form and Family Health Statement can be obtained through our Benefit Forms section. Select the form you need under the appropriate carrier.

I just had a baby. When does he / she need to be added on to my policy?
You will have up to 31 days after the birth to add your baby to your policy or your baby will not be covered.

When can a dependent be added to a policy?
Dependents can be added to your policy during an open enrollment period or within 31 days of a qualifying event. Qualifying events can include loss of other coverage, birth or adoption of a child, or marriage.

What do I do in case of an emergency and I am out of state?
Call 911 and/or go to the nearest hospital.  If it is an emergency, it will be covered as an in-network emergency visit or as an in-patient hospital admission. 

What if I am out of state and sick (non-emergency)?
Call the 800 number on the back of your ID card to determine a participating provider in the particular region.

Can I get maintenance prescriptions mailed to me and pay less?
Yes you can. This will save you both time and money. Through prescription mail order, you will typically save one co-pay every three months. Prescriptions will be mailed directly to your home.

One of our employees just gave their notice of termination and wants to continue their medical benefits. What do we need to do?
Complete an enrollment/change/termination form for the applicable carrier.  Fax or mail the completed form to the appropriate carrier.  You will also need to offer them COBRA. You can retrieve the enrollment / change/termination form from our Benefit Forms section. COBRA forms can be retrieved from our Human Resource section.

How do I change my Primary Care Physician?
Generally you can call the (800) number on the back of your ID card and change it right over the phone. You can also go online and register as a member and make changes.
